Business Associate Agreement (HIPAA)

Last updated: 24 July 2026

Draft for review. This document is a template prepared for Planquora and has not yet been reviewed by legal counsel. It is provided for transparency and is not a substitute for professional legal advice.
Important: Planquora does NOT currently offer a Business Associate Agreement and is not configured to handle Protected Health Information (PHI) under HIPAA. Do not use Planquora to store or process PHI.

A Business Associate Agreement (BAA) is required under the U.S. HIPAA rules when a vendor processes Protected Health Information on behalf of a covered entity. Planquora does not currently offer a BAA.

Current status

Planquora has not completed the controls, certifications, and contractual commitments required to serve as a HIPAA business associate. We therefore do not sign BAAs at this time, and the Service must not be used to store or transmit PHI.
